RE-ENROLLMENT STEPS

Step 1: Online Application
Complete the Online Application.
Step 2: Submit Documents
Submit the following documents to complete your Online Application (upload link will be online):
- Proof of Parent/Guardian's income - tax return, 3 current paystubs, SNAP/TANF letter, etc.
- Up-to-date Immunization Record (7th graders only)
- Medical Treatment Plans (if applicable)
Step 3: Pay Registration Fee
Pay the $25 annual registration fee, which confirms your child’s spot for the summer and is non-refundable.
Step 4: Attend Family Meeting
Join our Horizons family online this spring for important summer updates and preparation tips. A link will be sent to your email and/or via text message to your phone. Ensure we have your current contact info.
